Health Equity Alliance of Rock County
The Health Equity Alliance of Rock County (HEAR) is a local coalition working to advance health equity throughout Rock County by reducing health inequities. HEAR is made up of an Advisory Committee, General Body, and Subcommittees with members from Rock County Public Health (RCPH), local healthcare systems, school districts, nonprofits, and other organizations.
Vision: We envision a Rock County where everyone has opportunities to thrive and maximize their health.
Mission: Through community partnerships, we identify opportunities to remove barriers and advance health equity in Rock County.
Commitment: To create and nurture a space of integrity, humility, authenticity, and respect.
Our Work in Action
HEAR works to advance health equity by leading the development and implementation of Rock County's Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). To view the most recent data and priorities, view the Healthy Rock website.
HEAR also works to advance health equity through:
- Working with partners and Rock County residents to organize projects or programs that encourage equitable, evidence-based practices
- Reviewing local, state, and national data to inform and improve local health equity efforts
- Hosting focus groups and interviews with Rock County residents to better inform projects or programs
- Raising awareness of the root causes of inequities, including the Social Determinants of Health, and their impact on health outcomes
- Organizing community partners to share information and resources for improved coordination of community services
Impact on Rock County
HEAR is working to advance health equity to help Rock County reach its full health potential. Some residents are at higher risk of health inequities, and as a result, may have less access to healthcare resources and experience negative health outcomes like lower life expectancy and higher risk of chronic disease. Advancing health equity can benefit all of Rock County by reducing healthcare costs and improving productivity. HEAR's work continues to highlight the impact of health inequities to help find solutions and remove barriers that prevent Rock County residents from reaching their highest level of health.
