What does Birth to 3 do?

• Serves infants and toddlers with developmental delays and disabilities. 
• Provides comprehensive support as defined by an individualized plan created in conjunction with parents to help address their child’s developmental needs.
• Delivers services through primary coach approach in natural learning environments.
• Assists with transitioning children to the local school districts when they age out of the program.

Program Eligibility

• Screening (Complete if not auto-eligible or if physician hasn’t completed one recently)
          - Ages and Stages Questionnaire (ASQ-3-SE) & Ages
          - Stages Questionnaire - Social-Emotional (ASQ-SE)
• Eligibility is based on a diagnosed disability, 25% delay 
  in one or more areas of development, or signs of atypical development. Early Interventionists will evaluate the child’s cognitive, physical/motor, speech and language, social and emotional, and adaptive development.

Parents Role in Birth to 3

Parents’ active participation with their child is key. They know their child and family best: their routines and favorite family activities, interests, and abilities. 

We know from research in child learning and development that:  

• Everyday activities occurring in the family home or community are the sources for a child’s learning opportunities.
• Children learn best during activities that they find fun and interesting.
• Families feel more fulfilled when early intervention services are based on the child’s and families’ needs and family identified priorities.

How do you find learning opportunities by using natural learning practices and having fun?

• Each family is unique. Use opportunities in existing routines and what families prioritize. 
• Use everyday activities such as diaper changing or mealtime.
• Use a child’s interests such as toy cars or motion songs. 
• Affirm a parent who is showing responsiveness to their child by smiling at home, offering praise such as ‘good jumping’, and looking them with twinkling eyes resources when appropriate.

Primary Coach Approach

The Primary Coach supports the family in turning their daily activities into learning opportunities through coaching, modeling, and information sharing. 

• Physical Therapists 
• Occupational Therapists 
• Early Interventionists
• Speech Therapists 
• Early Childhood Special Education Teachers

UCP professionals are available to support all areas of development and concerns as needed. 

A Service Coordinator is assigned to each family to assure their plan addresses the family’s goals and to provide additional resources when appropriate. 

Transitions

A transition or exit plan must be developed with a family when all children exit the program. 
If the child is still enrolled in the program at age 3 and they are potentially eligible for special education services, the child is referred to local school districts or Early Head Start and a Transition Planning Conference is held to discuss next steps.
